Getting Started on a Stake/Ward preparedness survey
What has your ward/stake done regarding preparedness surveys?
Please share your thoughts, ideas, etc.

We had a questionnaire that asked the following (I don't have a copy of the survey, so I'm going off the plan that resulted from the surveys and my so-so memory of filling it out):
Name, physical address, phone numbers (home, cell, work)
Special medical needs.
Any special skills. They didn't give suggestions, so I think some people might have left some skills off. These are the ones that were reported, you might offer a similar list and have a space for "other":
Electrical, plumbing, computer, hvac, welding, gardening, mechanics, carpentry, accounting, farming, sewing, heavy equipment, masonry, first aid, nursing.
Equipment you have that would be available. Again, they didn't offer a list to jog people's memories, but here's what was written:
Chain saw, Welder, dump truck, air compressor, generator, tractor, portable water pump, cutting torch, horse trailer, horses, pickup truck, small hand tools, backhoe, kerosene space heater, motor home/trailer, table saw, cement mixer. If I were putting this together, I'd take that list and expand on it--maybe do a separate section for transportation options like trucks, horses, atv's, etc.? and still have a space for "other" so people could add things I hadn't thought of.
Are you required to help the city/community in another capacity during an emergency? If yes, where? We have highway patrolmen, a school principal, CERT members, and administrators of the Nursing Home in our ward, so in the written plan there is a page telling where those people will be needed in the community.
This survey was handed out during church and they gave us two weeks or so to fill it in and get it back to a member of the bishopric. You might get more back if you ask them to fill it out before leaving church, but maybe going home helped people remember what equipment they had. I was not on the committee for this survey, so I don't know what their response rate was.

I've tried several surveys over the years have had a very paltry response. Disappointing to say the least. I have, however, had good luck asking one question at a time. Usually I couple it with something in the ward or area that is relevant. For example, our last big rain happened to be on a sunday during church...Midway through sacrament meeting the High Priests started peeling out of the chapel one by one. We found out later that there were several new homes in the ward that had 5 inches of water in the basement and they were going to muck it out. We now know who has pumping equipment available for a situation like that. When it's real to them, they'll usually give up the info about skills and equipment.
